In this section you will find certain forms used in relationship to an ISACS team visit.
The Confidential Ratings Sheet (of visiting team leaders) is the same as Appendix P in the ISACS Accreditation Guide and is completed by each visiting team member on behalf of the visiting team leader. The Confidential Rating Sheet (of visiting team members) is Appendix Q and is completed by the visiting team leader for each of the team members. These completed forms are returned to the ISACS Central Office.
The ISACS School Profile (Appendix T in the ISACS Accreditation Guide) is completed by schools to accompany their self-study, reaction report and progress report.
The ISACS Disclaimer (Appendix O) is included by visiting team leaders in their visiting team report.
